The Applicant:

has read and agrees with the terms of the scheme of welders / operators certification, with the terms of payment, with the terms of using the certificates, with the terms of periodic confirmation/supervision.

NB! In the absence of periodic confirmation/supervision, the certificate becomes invalid.
The certificate is the property of the Certification Body and, upon suspension, cancellation of certification, the certificate must be returned to the Certification Body without additional conditions.

gives consent to the processing (including saving) of certification data and the publication of certified person's data on the Certification Body´s database. The data to be published is the name and surname of the welder, certificate number, validity periood of the certificate and the field of certification;

has the right to appeal or complaint;

has the ability to submit an application for taking into account special needs within reasonable limits (write in the additional information box).
